From developing a comprehensive care plan, to coordinating appointments among specialists, to simply checking in with families to see how they’re doing, our trained and compassionate nurse care managers are committed to improving the health outcomes of patients.
Where Unique Situations Are Understood
No two patients are the same and neither is the care offered by PANW. Whether your child has a chronic and complex healthcare need or has recently experienced an acute concern like a traumatic injury, our nurse care managers can help.
Our team collaborates with many children and parents who have medical complexities, such as:
- Cerebral Palsy
- Developmental delays
- Down syndrome
- Eating disorders
We also work closely with foster families and children who need an individualized education program (IEP) or 504 Plan.

- Check in frequently with families to assess needs and resources
